noise on start up

So let me start off by saying I never hear this noise until my son went to start the truck, he turned the key, it went to start and he backed off the key for a sec, and then hit it immediately again and it did a slight grind. Well now when you start it it seems to have a little tingy rattle on the passengers side for 5 seconds or so. then it goes away. Now if you shut if off and start again, I don't hear it, let it cool for 5-10 minutes and I hear it again. It always goes away but always on a cold start. I have done a bunch of searching in here and people lean to the timing chain tensioners. Maybe it made a noise prior and I just never paid attention but not sure I would of missed this sound, it is rather loud. only on the passengers side btw. I'm going to get a stethoscope to really try to narrow it down. Figured I would throw up a post and see if there was any advice out there. Truck has 105k miles on it. thanks

So did you go head and tear the front off, replace all the timing components right away? Is it a bitch? Would it just start out of the blue like that?
My first guess would be the starter, especially since it stops after a few seconds once started.
Yeah I was thinking that, but not sure why it would do it quite that long and seems if u give it a little rev in the first few seconds it is worse

Yeah, took everything off with the help of a couple youtube videos.. wasn’t terrible but I would hate to have to do it again lol

I think it started off fairly quiet.. eventually got louder as months went by, that’s when I decided to tear it apart
So after starting it and try to located the noise because only happens cold, I found the culprit. The damn back two bolts on the exhuast manifold are broken off, from the looks of it have been for some time now. So when you first start up the truck it rattles for about 5-10 seconds and then just goes away, not sure if it heats up and expands and stops it. Anyhow, now another whole bag of worms. From the looks of the other bolts, those things will probably break too. How the hell am I going to get those all out :(
This most definitely sounds like a broken timing chain guide.
